worden lamp form with pattern G19 1/2

Description

This is one of the Worden lamp forms where they still supplied the Styrofoam form with the pattern printed on the surface. Two forms for a 3 repeat lamp.  I've made two lamps off this form. This has been covered with silicone to resist heat damage. I didn't use the side pieces, I made a fixture to hold the two forms together and made the connections over the gap. All the pattern pieces are in numbered envelopes
